Abstract

An Apple a Day is a start up social enterprise and Community Interest Company whose mission is:

To provide convenient, accessible, tailored and evidence based health education to empower people living with Long Term Conditions, enabling them to become active and informed partners in their health care.

An Apple A Day is based in Nottingham and will commence operations by providing structured education for people with Type 2 diabetes. This education will be part of the diabetes treatment pathway, and will complement existing health care services.

Structured patient education increases patients' knowledge of their condition and empowers them to change their behaviour so they can better manage it and have healthier lives. There is evidence that the education programmes we provide will help self care and improve biomedical outcomes, as well as improving patients' quality of life and sense of control over their condition.

An Apple A Day will extend its portfolio of products to include education for health care professionals, and self management courses and education for people living with other long term conditions such as heart disease and asthma. An Apple A Day will develop its services in conjunction with its stakeholders in order to meet the health needs of the local population and reduce inequalities.
